[mythtv-users] changing channels & other errors

I have copied the backend log when tried to change channels tonight. I 
got a similar readout to the below on my frontend i.e. waiting for 
decoder to pause, unpause, then seg fault.:

I have read on a few threads that a rescan will cause this error to go 
away.  However, I have deleted all tuners/inputs/sources and the above 
was done with only one tuner setup in mythtv-setup.

Any ideas on how to fix this "Wrong PMT" error?

My setup in short: Gigabyte 8i945gmf-rh mobo; Pentium D 925 CPU; using 
onboard GMA950 graphics to large old CRT via VGA (still works ok...WAF 
increases with larger newer thinner tv for some reason); Leadtek 
dtv1000t PCI DVB-T tv tuner; DVICO HDTV Fusion Lite PCI DVB-T tv tuner; 
onboard analog 5.1 audio output to 5.0 Yamaha Sound System with alsa 
plug; 300 watt no-name PSU; Silverstone LC17 HTPC enclosure; Archlinux 
64-bit updated today...; Running combined FE/BE.

I still don't know why my system is sometimes unresponsive and I cannot 
change channels up or down using up/down arrow keys (or remote up/down 
keys for that matter)...
