mplayer2 is a fork, but I don't know that it does manual frame step.

Depends on what you're trying to optimize for; bidirectional varispeed 
scrub was exactly the thing I was looking for as well, and it's apparently
pretty difficult to do well and generally with MPEG source material.

If the added overhead of Myth to get that is tolerable for you, then...
