This usually means that the guys at the transmitter ends moved the goalposts on 
you. That is, the channels currently being output in each of the multiplexes 
aren't the same as they were when you last scanned. Something got moved. Either 
there's a new channel myth doesn't know about, or there's a channel myth expects 
to find in a particular multiplex which is no longer there. (Could be multiple 
channel changes. In the UK, Freeview does this every few months. I expect this 
to continue until things settle down sometime after 2012.)

It invariably means a rescan. This might clear up some of your problems (though 
possibly, not all of what you described).
