I don't see anything in the apache log. Here is the access, error, and

It also looks like the modules_path should be set in defines.php but I
don't see any error.

Ultimately, I had some permissions issue which I have solved.
However, modules_path is defined after the set_error_handler is called
includes/errors.php. So, any errors that happen before the modules_path
is defined will not get logged properly. This seems like a bug to me.
