The official (but cumbersome) method to remove unreachable cards is to
choose the "Delete All Capture Cards" option which truncates the table
in the database, removing all cards and resetting the card numbering,
and then reconfigure all existing cards and input connections.

The unofficial (quick and yet potentially disastrous so take a DB
backup as a precaution) method is to stop mythbackend, remove the
entries* directly from the database, and then restart mythbackend.
This leaves all other capture cards and inputs configured, and can be
effected in a few seconds.

* the relevant tables are 'capturecard' for the physical card, and
'cardinput' for the input connection. If it's a multirec card or
multi-input card, you may also have enties in 'inputgroup'.
