The 'export DEBUG_PULSE_AUDIO_ALSA_EMULATION=1' immediately prior to
running mythfrontend fixes the problem for me.

To everyone who is suggesting other remotes the reason I HAD to fix
the problem without buying a new remote is the system is also driven
by a Sony PS3 bluetooth remote which gets used during the day for
music playing around the house.  However when the phone rings the
volume needs to be turned down from wherever, the easiest way to do
this was with a BT remote so that's what I did.  WAF very high as it's
small and neat, big clunky complicated remote that needs to be pointed
at the Amp as a replacement would have been a non starter.

I'm a happy bunny at the moment though as the system does exactly what
I want, 1 remote controls the volume all through the myth system...
