 From your original irw printout, it looks like your remote or receiver 
is stuttering somehow.
the first column is the remote code, the second is the repeat number.

Its impossible to press a button fast enough for only one pulse to 
output, but the repeat and delay items in lircrc make it respond properly.

Check your remote with a digital camera, webcam or even a cell phone 
camera - they can sometimes see enough IR to make sure its working. also 
check with your HDPVR remote.
There is also the program xmode2 that shows a graph of the remote code. 
It may help show if the IR signal is dropping out.
