I don't ever check Forced Only.

Also, the video that you've already ripped.  Try pressing t on the 
keyboard while it's playing.

My saved settings for Myth are:

     H.264
     Same as source
     Constant Quality 16

[audio]
     English (AC3) (5.1ch)
     AAC (faac)
     224 Biterate
     Sample same as source
     Mix Stereo
     DRC off

[subtitles]
     1-English (VOSUB)

[H.264]
     set to defaults

[Picture settings button]
     Auto crop and Loose crop unchecked
     Cropping all set to Zero
