My solution was to add the following to /etc/modprobe.d/blacklist:

blacklist ivtv
blacklist pwc
blacklist bttv
blacklist cx18

(pwc is for a webcam, but it uses a /dev/videoXX device file.)

This way the modules for the capture cards won't be auto-loaded.

Then, I manually add the modules in the order that I want by specifying
them in /etc/modules:

ivtv
bttv
cx18
pwc

YMMV with USB capture card; I have never used those.
