I'm seeing a failure to tune both inside and outside of Myth (dvbscan). "Timed out, no signal"
displayed for each channel.

Is this the classic mode of failure for the HVR-4000 and kernel v2.6.30-32?

I've installed firmware v1.23.86.1 (v1.26.90.0 appears to no longer be available and I've been
unable to track down instructions on how to extract the firmware from the latest
88x_2_125_28070_WHQL release from Hauppauge).
