I'd fire up xmode2 and and look at the waveform. More importantly, I'd look to see if you get a waveform(s) without pressing a button. Could have a loose connection or a bad solder joint. BTW, I've noticed that irw can have a tendency to pop out unexpected results. May want to try with myth to see how it works.
