I've been using Bell ExpressVU (Canadian, but uses the same hardware as
Dish Network in the US) for many years now with no issues. I have a
PVR-150 MCE (with the ir blaster) connected to a SD receiver via SVideo,
and I've recently (basically, when 0.22 came out and the HD-PVR support
hit stable) set up my HD receiver connected to the HD-PVR via component.
I use the IR Blaster that came with the PVR-150 (it's the USB unit,
not the one that plugs in to the back of the PVR card) to control both
units (they're set on different remote channels).

It would be nice not to need the receivers, but everything works very
well, so I won't complain. It's a heck of a lot better than the
Dish/ExpressVU PVR solution...
