I think in most (if not all) current cases, it's actually just a broken
OSD theme. Yeah, I saw where the OP said he tried all possible OSD
themes in existence...
Then again, I don't remember if the
offset-because-MythTV-is-not-run-fullscreen issue was ever fixed or if
it's not being fixed because mythui will completely replace the code.
"Not-run-fullscreen" means run with offsets or width/height or (in
MythTV) overscan.
