TwinView enabled (perhaps without your knowledge)?  Xv is only supported
on the "primary" display, so it sounds like you have TwinView or some
other Xinerama(-variant) and would get video on a monitor connected to
VGA while getting a blank screen on the TV.  (Of course, if the
VGA-connected monitor were to change the autodetection sufficiently to
change the Xinerama configuration, the process of testing may prevent
you from seeing the expected results...)
