I have a MythTV (FE/BE and two separate FEs) and two TiVos, an
S1 and an HD. 

The MythTV is harder to install, but if you bought one off the
shelf like you do a TiVo, that wouldn't necessarily be the case.

The MythTV needs maintenance by a skilled admin. The TiVos do
not. The MythTV is more flexible and upgradable and all manner
of good things.

Operational difficulty is very, very similar. Anyone who can
learn to run one can learn the other.
