Just copy the VIDEO_MANAGER <button> definition from 
/usr/{,local/}share/mythtv/util_menu.xml to, i.e., 
/usr/{,local/}share/mythtv/themes/DVR/util_menu.xml .  Don't know the 
answer to why.
