I've never introduced a bug into a piece of software I was QAing in my  
life because I've never checked in code into software I was QAing.
Ironically, Microsoft has one of the best Test Engineer to Software  
Engineer ratios in the industry. I think the problem is that their QA  
department writes all their automated tests in some C variant (C#, if  
I remember correctly) and their automated test code is about as good  
as their release software code.
