I used to have this problem some time ago.  It my case it was because
MythDVD was ripping (to ISO) only the first 4G of a two layer (8G) disc.
See that the sizes of the rips are what you expect.  Also try ripping
with something like ddrescue instead of MythDVD and compare the results.

It also happened when playing non-ripped DVDs.  For whatever reason it
couldn't always read that second layer.  Sometimes cleaning the disc
would resolve the problem.  

I should say that I haven't had this problem in nearly a year.
