I have had this issue in two scenarios. 1) My tuner freaked out during
channel change but failed to tell Myth it failed yet hung up the device and
wrote no output file (requires a reboot to fix the tuner - PVR-150) and 2)
I've had inodes "go bad" on my disk and the repair had to delete the file
but Myth was blissfully unaware. In either case I had to delete the
recording from with-in Myth and set it to re-record at a later date/time.

You're not alone on House... My backend freaked out on both of the final
episodes "House's Head" and "Wilson's Heart" I caught the first one last
week on USANetwork and the second one is also showing on USANetwork June 6th
11pm. If I don't catch this one I'm gonna have to buy Season 4 on DVD.
