I believe this is just a PCI bus problem.  I have the same card and 
after upgrading to trunk and using VDPAU it all goes away.  It is the 
only way I was able to fix it and I spent quite a few nights updating 
everything, flipping options and nothing worked. Even SD video had a 
problem. 
It works beautifully now, 7% CPU 1080i with one-field deinterlacing (I 
was able to use 2x basic before, but with the latest nvidia drivers 
180.16 and latest mythtv it stutters).  The OSD also causes the CPU to 
spike and video to stutter, but I am sure that will be fixed as the code 
gets more mature.
