On some systems, it's a bios thing.  If the bios sees a connection at the
VGA/DVI out, it turns it on and disables TV out and vice - versa.  I don't
know if any of the nVidia cards implement this "feature".  So first make
sure that you can get output on both at the same time, then set up two
different xorg.conf files and switch between them.
