Here is my VNC setup:

x11vnc -forever -rfbport 5900 -rfbauth /home/mythtv/.vnc/passwd

That allows me to connect on port 5900 via VNC using the password in the
.vnc/password file to authenticate (details on password file creation here
http://www.karlrunge.com/x11vnc/ ).

One thought regarding your issue -- are you setting the TV size to 640x480
or letting nvidia handle it? I've found with my Nvidia->SVideo->27" CRT 4:3
TV that setting any screen sizes decreases performance by ~ 6-10%. If I let
nvidia do it, I get a 1024x768 virtual screen which the card renders down to
NTSC and shoots out the svideo port. I couldn't tell you if it is
progressive or not -- everyone here always replies "oh yeah, a CRT -- you
shouldn't need to do deinterlace" and then a bunch of CRT users point out
the various bugs and gotchas and basically explain that they need to
deinterlace if they want a halfway decent picture. I've experimented with
both options and basically use OpenGL vsync + deinterlace now. Every once in
a while I try it without deinterlacing but there is always a gotcha in my
experience -- hopefully, you have better luck.
