Build a time-machine and get MythTV version 0.22. I'm pretty sure
Stuart Morgan is working through that and making things more
consistent. I don't remember if he said he was switching all of them to
keyboards or making it a user-level option or making it a themed option,
but I remember his mentioning it.

When you do, I'm sure the devs would very much appreciate a diff, as it
will allow them some time off (unless, of course, you find out that 0.22
needs a lot of work).

I thought everyone knew 9 = 0... ;) (OK, in truth, I can't even
remember if that's right, so I feel your pain.)

Maybe you were thinking of 0.22, but you remembered it in the past
because of your time machine?

Unfortunately, short of changing the code in Myth (which will be thrown
out before 0.22 during the mythui conversion), I don't think there's a
way to modify 0.21 as you desire.
