If I understand properly the bad combination was XFS + LVM + software  
RAID while using 4K stacks.  The problem was the number of layers --  
it made for a deep call path that overflowed the smaller stack.  In  
theory you could get into this kind of trouble with any filesystem if  
you stacked up enough block device layers underneath it.

No idea if it's fixed.  I think 4K stacks are still considered an  
experimental feature in the kernel, but some distributions (e.g.,  
Fedora) have chosen to enable them.  There's some pressure in the  
kernel developer community to move to 4K stacks as the default, though.
