It sounds like you are looking for something very similar to the
silent frontends that I have built.

They are based on a Via ME6000 motherboard.

If you want a silent system that's a backend as well, the noise from
the hard drive is an issue, and cooling is more of a concern one you
start packing a tuner and hard drive into a small case.

If you're after true silence, other than running long audio and
monitor wires to somewhere else in the house, I think the only option
is a network attached backend out of ear shot.

I system like the ME6000 certainly has some trade-offs in terms of
performance, but can truly be silent, and is very energy friendly.  (I
hope you don't want HD.)
