Add the following lines to */usr/local/share/mythtv/library.xml*

or for the "classic" style add the lines to 
*/usr/local/share/mythtv/musicmenu.xml*

   <button>
      <type>STREAM</type>
      <text>Internet Streams</text>
      <action>PLUGIN mythstream</action>
      <depends>mythstream</depends>
   </button>
