This is a known issue, it is logged in Trac, the bug number is 2563
(http://svn.mythtv.org/trac/ticket/2563).  It has also had a couple of
other ticket numbers as well, I don't recall them offhand.

I see it more with HD than SD (perhaps due to the increased bitrate), but
it can happen in SD also.
