It's generally a process of elimination.

First: Are you absolutely positive the problem was in fact on the DVD,
and not some sort of playback problem? Was it totally repeatable? Did it
happen on multiple players?

If it was a problem on the DVD, was it repeatable? If you burned another
one did you have the same problem?

Of course, if you removed the spots from the file you may not be able to
go back to the original.

Often trying to track down this sort of thing is futile, but you did
what I would do, check the list to see if anyone else had had similar
problems.
