I had the same problems with a pvr-350 + 150MCE, first using the 
HcwMakoC.ROM for v4l-cx25840.fw gave me a totally mute 150MCE as second 
card,  

ivtvctl --set-ctrl=mute=0 -d /dev/video1 
did not change the output:
ioctl IVTV_IOC_G_DRIVER_INFO ok
ioctl VIDIOC_S_CTRL ok
ioctl IVTV_IOC_G_DRIVER_INFO ok
ioctl: VIDIOC_QUERYCTRL
Brightness = 128
Contrast = 63
Saturation = 63
Hue = 0
Volume = 60928
Balance = 32768
Bass = 32768
Treble = 32768
Mute = 1

Mute was always 1, even if trying to set it to 1 and back to 0. even 
tried "v4lctl -c /dev/video1 volume mute off", but no changes. also not 
fiwth "F9" or mute button on remote on the second tuner in mythtv,
then I switched to firmware HcwMakoB.ROM, and rebooted (hot), and it 
got set to Mute = 0 per default and all was ok. I just switched the 
firmware back to HcwMakoC.ROM and it _still_ had mute = 0, I haven 
tried coldboot yet. But maybee there is something in conjunction 
with ivtv 0.7.0 and firmware HcwMakoC.ROM that doesnt initialize things 
right? the first boot with fw HcwMakoC.ROM was cold with this setup.

I'm currently recording stuff on both tuners, so I can't debug this 
further right now, but if required, I would be happy to try some more 
debugging tomorrow.
