I have been struggling for a few weeks to get my NVIDIA TV-OUT working, and finally did so at the weekend (that's a whole different story of stupidity and RTFM).

I've set the TV picture to be a section of the picture the normal monitor-out sees (specifically, the upper-left portion).  The TV 'monitor' is set at 640x480, and I've set the FE to run at that resolution. 

With the FE running, all I see on my TV is Myth, but what I see on the monitor is exactly what you describe - Myth FE runs in the upper left corner of the screen, with the KDE desktop behind it.  It really does sound like something as simple as the FE resolution (isn't it always?)!
