Ok, just got through trying this, with no luck:
1) dropped the mythconverg database.
2) recreated the database per the docs.
3) Started mythtv-setup, and set up the card and it's two sources.
Created an "antenna" source, which is connected to the tuner input. I
selected "fetch channels from lineup", and that worked fine. That is,
the option where you set the initial channel for that source listed all
the antenna channels from that lineup in Datadirect.
Then I set up the 4DTV source, which is connected to the svideo input.
Fetching the channels from Datadirect only seemed to retrieve the
channels for one satellite, as seen from the initial channel selection.
4) Ran mythfilldatabase --do-channel-updates, and let it chug a bit.
When that finished, I started the backend, and frontend. The EPG
displayed all the antenna channels, but only the 4DTV channels initially
found by mythtv-setup.

So, no joy on that. I did add a "-v all > debug.txt" to both the
mythtv-setup and mythfilldatabase. Looking through the output from
mythfilldatabase did seem to indicate that some part of the process was
receiving all the 4DTV channels. There was a large section of "insert
into dd_station" lines that listed all of the 4DTV channels. I don't
know too much about MYSQL or the myth database layout (I have experience
with Oracle and Postgresql), so I haven't tried looking through the
database for the missing channels.
