Oh no, no appologies needed, you *did* help, by kicking my thought
processes down another path. Like most problems it's knowing the right
questions to ask, I spent ages looking for a solution and got nowhere.

TBH I play DVD's through a standalone player and not attempted to play
them through mythtv, I just use the audio out for playing videos and
music.

Hmmm, this did give me an idea though, some looking later and I have more
of an idea, files recorded on the 'old' mythtv-0.18 are either

# file 1004_20060902041000_20060902051100.nuv
1004_20060902041000_20060902051100.nuv: MPEG sequence, v2, program multiplex

or

# file 104_20060724000000_20060724010500.nuv
104_20060724000000_20060724010500.nuv: MPEG sequence

whereas files under mythtv-0.20 are either:

# file test1
test1: TeX font metric data
(\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377\377)
# file test2
test2: MPEG sequence, v2, program multiplex

Soooo, that's not MPEG, at a guess a transport stream, hmmm Transport
Stream eh, so more searching finally lead to
http://svn.mythtv.org/trac/ticket/222

Which says that TS->PS was turned off for mythtv-0.19 (which I skipped)
which then lead to
http://www.gossamer-threads.com/lists/mythtv/users/152464

So the answer is simply to transcode or use a pass-through cable.
