By any chance can you verify whether your HD original stream is "clean"
or if you have bit errors in it? I see the same problem with MPEG-TS
streams captured off DVB-T, and these regularly have noise in them that
breaks some of the packets.

These also cause errors to be logged during commflag, transcode and
playback: 

[mpeg2video @ 0x104abc4]get_buffer() failed (stride changed)

[mpeg2video @ 0x11d2bc4]skipped MB in I frame at 1 21
[mpeg2video @ 0x11d2bc4]ac-tex damaged at 0 22

etc
