Try adding repeat = 3 in there. Not sure why this helps, but I have  
it all over my lircrc file and it's happy.
I know this sounds dumb, but also make sure you've got your lirc  
config in ~/.mythtv/lircrc and symlinked to ~/.lircrc so it'll work  
in both myth and xine.
