How are you calling "mythtv-setup"? From the console? Or from a terminal
window whilst logged into a window manager?
Is mike a root user? Does mike have full permissions to his home directory?
Have you ever run mythtv-setup from mike's home directory as a su'd root
user?
Does a mysql.txt file exist in the current home directory under the .mythtv
folder? What are its permissions?
