As is ever the way, just after posting this I found the solution. 
Problem is with the new ATI drivers - see bug here

Moving back to 8.20.8 fixed the problem - TV out is now working fine. 
Hurrah!

Contrary to what is said in the comment to the bug, this driver works 
with a 2.6.15 kernel.

Thought I'd update to the list in case anyone else was seeing the same 
problem.
