I think this is probably normal.  They have said numerous times on the IVTV 
list that because the driver is mostly reverse-engineered, they do not have 
the capability to correctly initialize everything on the board, so data is 
retained in a warm boot, whereas a cold boot will flush everything out.  On 
my system, if I do a warm boot after a successful load of ivtv, then it still 
seems to load again just fine, so I'm not sure why you get that behavior 
unless the warm boot was after upgrading to a different driver version.
