Hi Steve, 
   Having gone through this myself, I feel your pain.  

My advice is focus on getting the TV-Out working using the IVTV tools
(be sure to set your output to S-Video using ivtv_fbctl or it's
equivalent)  working first, then worry about Myth.

Looking at the output you posted, your modules.conf appears to be in good shape.

You've probably seen this already, but I found the HowTos on the IVTV
wiki quite helpful:

Also, check the IVTV mailing list - you're much more likely to get
answers there than here:
http://www.gossamer-threads.com/lists/ivtv/devel/

I have some notes on the steps I followed to get SVid out working - if
you specify the driver version you're using I'll see if they may apply
to your situation.
