If the gray remote that came with your PVR-250 is the same gray remote that
came with my PVR-350, your configuration is wrong.  (IIRC, I ran across one
or two bad config files before I found the correct one.) Here's what I'm
using:

Try that and see if it works any better.  You also want to make sure lircd
is running, or else irw (and MythTV's internal LIRC support) won't work...if
you haven't done it already, do this:

rc-update add lircd default && /etc/init.d/lircd start
