This all sounds like a mixer problem.

alsamixer

Get a shell running on the machine while you can hear the TV playing
through it.  Run alsamixer, and find the input corresponding to the TV
audio.  On mine, it's "Line".  Set the volume to 0.  You'll know when
you've got the right one, cause the sound will go away.  Once you've
found the input and set the volume, you want to set that input to
"capture" (with the space bar).

Once that's set, you still need to adjust the recording level.  For my
setup, there's a separate setting labeled "Capture" (is it the same on
all devices?), which is essntially input gain.  I have to set this
really low  (looks like it's currently at 13%) to avoid clipping.

Assuming your PCM and Master levels are ok, that should be all you need to do.
