I always used this trick with succes, but I haven't the foggiest about why:

#!/bin/sh
/usr/local/bin/test_ioctl -v input=7,output=1
/usr/local/bin/test_ioctl -v input=2,output=2

Run this code well after you modprobed ivtv, or it will go croocked.
