This is a total stab in the dark, as I didn't read the rest of this 
thread, but is something leaving an open file descriptor on your box in 
that FS?  Is some process running away and filling a "file" even though 
that file doesn't really exist any more?  I've seen this on systems 
before.  Once the process is cleared and the FD is released, all is 
usually well.  An fsck would probably fix any allocated inodes.
