You can grab master_iconmap.xml from:

http://cvs.mythtv.org/cgi-bin/viewcvs.cgi/*checkout*/mythtv/contrib/master_i
conmap.xml

You can grab mkiconmap.pl from:

http://cvs.mythtv.org/cgi-bin/viewcvs.cgi/mythtv/contrib/

Depending on where you live there is a chance you need nothing more than
master_iconmap.xml.
