One thing that came to my mind when I was reading this was something I
discovered when I first tried got X to display on the PVR-350 output.  I
set things up and start X and all I get is a black screen on the TV
and my normal desktop on the monitor.  However, if I move the mouse
over to and off of the monitors edge, onto the TV display, I get the X
cursor on the TV.  I use XFCE4 as a window manager and there's
probably a way to tell it to manage the second display, however you
don't really need to.  By starting mythfrontend as
>DISPLAY=:0.1 mythfrontend
I get mythtv on the TV.  You'll probably need to play around with the
UI size and offsets to get it to look good.  You also need to specify
the PVR-350 output under the TV settings.

Also, it might be worth trying Matthias Badaire's alternate X display
driver for the ivtv framebuffer at http://membres.lycos.fr/badzzzz/
