Problem solved.  Cox Cable uses "HRC" frequencies.  Changed 
~/.ivtvrc FrequencyTable= to ntsc-hrc

Maybe this could be mentioned somewhere?  If the default 
ntsc-cable doesn't work, try ntsc-hrc and how.
