What version of ivtv are you using exactly? Since you have the cardtype 
parameter, I suspect it's not 0.1.9. If it's one of the latest 0.1.10 ck 
  patches, then you can specify tuner=x,y as ivtv option instead of 
specifying type=x as tuner module option. If you are using 0.1.10 pre2 
without patches, then try this:
