I used to have two bt8x8 cards in my box and I used the same basic
format:

#options bttv card=10,40 tuner=21,2 radio=1,1

The only thing I can think of is that the order of the options makes
some difference.  I recall having problems getting mine going together,
but don't recall the details.  Are you sure you are specifying the right
tuner number?  Is the first card detected the card=10, tuner=21,
radio=1?  If not, try switching the cards around or changing the order
of the options line.
