XvMC only accelerates MPEG-2 decoding on video cards that support it.  
The answer to your second question is by getting the MythTV sources 
(either release or CVS) and editing the settings.pro for the MythTV 
directory and uncommenting the section related to XvMC and recompiling 
MythTV.
