Ray is right:

I had the same issue with PVR-250 and my DirectTV.

You need to execute (and I think I remember the right path here) 
/usr/lib/ivtv/ptune.pl (or, for a little gui, ptune-ui.pl).

For me, setting input to 'Video 0', and then selecting 'channel 3', and 
then hitting '3' worked.
