If I understand correctly, you're not supposed to :-)

The ivtv cvs has changed ioctls that match those in mythtv cvs, but not
vanilla mythtv 0.12.

Last update to ivtv cvs is, wait a moment, 4 days old. That's the one I
tested. It's not what you would call a production driver.

Try mythtv 0.12 with Jens Axboe's ivtv patchlevel as of 291003.
